Social and Emotional Health Screening Project 2021
This QI project is designed to improve provider screening for social and emotional drivers of health in children under the age of 5. Through collaboration, providers treating children and adolescents will work together to improve screening, management and referral processes. Screening will be done using a family centered and strength based approach. This project is particularly timely given the effects we are seeing on young children from the coronavirus -19 pandemic.
What is the project?
- Establishing social determinants of health screening in children 0-5 years for at least one visit using the SEEK screening tool.
- Establishing social emotional screening in children 0-5 years for at least one visit (tools include ASQ-SE2, Pediatric Symptom Checklist, Strengths and Difficulties and ECSA).
Who is eligible?
Pediatricians, family medicine physicians, NPs, and PAs who work with children under age 5.
What do I need to do?
Participate in monthly, lunch time one-hour remote learning collaborative calls from June-December 2021 starting with a kickoff call on June 16 – 12:30 pm-1:30 pm.
These webinars will have expert speakers that will help teach providers about SDOH/SE screening
Upload data once a month regarding screening for children seen for well visits – starting with baseline in June, then July, August, September, October and November. We will train practices on how to do this
Participate in brief surveys before and at end of project.
